INITIATING A CONSULTATION OR TRANSFER/TRANSPORT REQUEST To initiate a consultation or transfer/transport request, call the page operator at 773.702.6800 and ask for either the perinatal or neonatal transport physician on-call. The on-call physician will discuss management of the individual case with you. Jointly, you will decide if transfer/transport is indicated. If it is, you will jointly decide the appropriate mode of transportation - by ground, by ambulance unattended (transfer), or by air accompanied by the transport team. The on-call physician will then contact the Perinatal Center transport facilitator who will coordinate the transfer/transport arrangements and the communication between hospitals. If you experience a delay or failure in the above procedure, you may page the Perinatal Center transport facilitator directly at 773.204.8414. Enter your phone number after hearing three beeps. If you have any concerns or questions about this system, contact Cathy Gray, Perinatal Center administrator at 773.702.3733 or 773.702.6800, pager 8110. |
